Medical Services

Medical

Medical care is provided by a staff of full-time physicians, nurse practitioners, physician assistants, registered nurses and additional support staff. Our medical teams provide diagnosis and treatment of health conditions, well-child care, immunizations, family planning, minor surgical procedures and preventative care. Oneida Comprehensive Health Division offers the following specialties: Internal Medicine, Family Practice, Pediatrics, Podiatry, Diabetes Care, Obstetrics and Gynecology. Care is also available in specialized areas through contracted relationships with physician specialists.

Using a team-based model of care, we consistently work to provide the best possible care in a holistic and culturally sensitive manner. We believe that communication, trust and respect are foundational to a healthy relationship with our patients.

Appointments

  • Clinic appointments are required, however, patients requiring same day service for acute illness will be screened by the triage nurses and appropriately referred for care.
  • Appointments to the specialists are accepted by referral only.

The medical care is open for appointments between the hours of 8:00 am – 4:30 pm, Monday thru Friday unless otherwise indicated.

NOTE: Patients who arrive more than 5 minutes late after their scheduled appointment time may be asked to reschedule.

Team-Based Care

The medical team at Oneida Comprehensive Health is phasing in a patient centered model of care called team-based care. This model of care places each patient at the center of a team of providers and staff who can help them address all of their health care needs from preventative care to management of chronic conditions.

Team-based care offers patients advantages including access to care, improved patient outcomes, stronger relationships with your care team and higher quality interactions with a team that knows you and your medical background. This higher level of care and better outcomes are possible because each team has shared goals, clear roles and effective communication all centered around you, the patient.

How are teams structured?
We will have team-based care teams for Internal Medicine, Family Practice and Pediatric medicine. Family practice will have more than one team because of its size. Each team will consist of:

  • two providers (physicians and/or nurse practitioners)
  • one registered nurse (RN)
  • two licensed practical nurses (LPN) and/or medical assistants (MA)

Each team will focus on a smaller population of patients, which will enable them to get to know you at a deeper level.
