Patient Portal

Health Portal

Welcome, and thank you for using our patient portal!
Would you find value in being able to securely message your doctor from the comfort of your own home or to pre-register for your next doctor appointment without having to stand in line at a registration desk? Oneida Comprehensive Health Division (OCHD) thought you might!

A team at OCHD has been working quietly behind the scenes to bring a new health portal to life to improve the patient experience. With the new patient portal, you can:
      ·  Have easier access to your medical information and your healthcare team
      ·  Securely correspond with your healthcare provider through the portal
      ·  Register for your appointments from your computer or other device
      ·  Find test results from home
      ·  Use your phone to access the portal with a new mobile app

The new portal will also be easier to sign up for. When you share your email with OCHD, you’ll receive a message once the new portal goes live inviting you to register.

Here’s how you can get your name onto the list for the new patient portal:
      ·  If you’re a current portal user:
           1)  Log into the existing portal.
           2)  Click “Update your demographics” in the lefthand column.
           3)  Select the family member you’d like to update the email address for.
           4)  Scroll down and enter your personal email address in both fields.
           5)  Click “Submit” at the bottom of the screen.

      ·  If you’re coming in for a visit, you can share your personal email address with the Patient Account
         Representative who is helping you.

The patient portal team expects to continue making new services available through the portal over time. We appreciate your patience as we transition to the improved portal and encourage you to share your contact information.

Dental Portal

Welcome, and thank you for using our patient portal!
Through the Dental Portal, which is independent of the medical portal, you are able to:
    – Send and receive messages from our office
    – Receive follow up messages from our your provider
    – View a summary of your dental visits
    – View future scheduled appointments

Optical Portal

Welcome, and thank you for using our patient portal!
Through the Optical Portal, which is independent of the medical patient portal, you are able to communicate with your vision care provider and review a summary of your office visits. At this time, the summary does not include glasses or contact lens prescription information. If you’re looking for this information, please contact our office at 920.869.4921 during regular business hours (Monday through Friday from 8:00 a.m. until 4:30 p.m.).

First time using the Optical Portal?
After clicking the link above, select “Not Registered” and then enter the same email address you provided to Oneida Comprehensive Health Division during an earlier visit.
