Oneida Comprehensive Health Division Introduces Team-Based Care

When it comes to your health, you want to know that you are your healthcare provider’s top priority. Who you choose as your healthcare provider can make a big difference in the quality of care you receive. At Oneida Comprehensive Health Division (OCHD) our goal is to give you and your family the education, attention and resources you need to live your healthiest life. We believe a holistic approach to healthcare is critical to our health as a community.
That’s why we’ve started implementing team-based care. Team-based care is a model that puts you, the patient, at the center of a team of providers who can help address all of your health care needs from preventative care to managing chronic conditions.
The model offers a variety of advantages including:
- Better access to care
- Improved patient privacy
- Stronger relationships and higher quality interactions with your care team
- Improved coordination and communications among all of your providers
- Improved patient outcomes
Team-based care offers a higher level of care with shared goals, clear roles and effective communication between providers and staff that is all centered around you, the patient.
You may have already started to see these teams at OCHD. As we continue to roll out this model, each patient will have their own team. Teams will generally consist of one registered nurse, two support staff (Certified Medical Assistant, Licensed Practical Nurse or Medical Tech) and two providers, one primary and one alternate (Medical Doctors, Physician Assistants or Nurse Practitioners).
Together each team will focus on a small population of patients, which will allow them to get to know you and your family at a deeper level. The teams are built to help meet both individual and community primary care needs and enable you to be a full participant in your own care.
