Applications and Forms

Completion of the forms authorizes the Y.E.S. Program Staff to work with the students, the school, and access some student database information. Through accessing the student database information, the Y.E.S. staff is better able to help the students by monitoring overall progress. The authorization is valid until revoked or for the duration of the student’s attendance at the school.

For students attending Pulaski Hillcrest Elementary, Seymour Rockledge Primary/Intermediate, Seymour Middle and High School, or West De Pere Middle and High Schools, both forms are required.

1. YES Parental Release of Information
2. YES Program Indian Student Eligibility Certification and certification and proof of tribal enrollment in a federally recognized tribe. If the student is not tribally enrolled, proof of parent or grandparent enrollment is needed.

For Green Bay Schools only, both Forms are required:

1. YES Authorization and Consent
2. YES Program Indian Student Eligibility Certification and proof of tribal enrollment in a federally recognized tribe. If the student is not enrolled, proof of parent or grandparent’s enrollment is needed.
