As Director, Lindsey will oversee a comprehensive network of programs and professionals dedicated to helping elders, adults with disabilities, and individuals living with chronic health conditions remain safe, independent, and connected within their homes and communities. The department includes the Tribal Option/Family Care Program, Home Health Agency, Home Care Worker Program, Dementia Care Specialist Program, Community Case Management and Medication Management, Community Health Representatives, medication delivery and medical transportation services, and long-term care nursing services.

The creation of the Continuum of Care Department reflects OCHD’s ongoing commitment to providing coordinated, community-centered healthcare services. By bringing these programs together under a single department, the Oneida Comprehensive Health Division is strengthening service coordination, improving operational efficiency, and creating a more seamless experience for individuals and families seeking care. OCHD has a long history of expanding services to meet the evolving needs of the community through a holistic approach that promotes wellness and supports healthy living.

Based in Oneida and serving community members throughout the reservation and surrounding areas, the department functions as the primary hub for coordinating in-home, community-based, and long-term care services. Staff members work closely with healthcare providers, nurses, pharmacists, caregivers, and support agencies to ensure individuals receive the right level of care at the right time while remaining in their homes whenever possible.

The department’s mission is to support Native American families through respectful, quality care built on pride, communication, and trust for a healthier community. Services are designed to address the full spectrum of care needs, from preventive support and wellness services to complex long-term care and home health needs. Through this coordinated approach, the Continuum of Care Department will help ensure that community members receive the support they need to maintain their health, independence, and quality of life while remaining connected to their families, culture, and community.

Lindsey’s appointment marks an important step forward for the Oneida Comprehensive Health Division as it continues to enhance access to care and strengthen services that allow community members to age in place and thrive in the setting they call home.
