COVID Vaccination Registration

COVID Vaccination Form

Oneida Nation is holding COVID-19 Vaccination clinics for enrolled Tribal Members and active patients of the Oneida Comprehensive Health Division.

COVID-19 Vaccine Eligibility Guidelines beginning March 1, 2021

Proceed if you would like to receive the COVID-19 vaccine and one of the following apply to you:

  • Oneida Community Members 18 and older
  • Oneida Nation employees
  • Frontline health care personnel
  • Residents and staff in skilled nursing and long-term care facilities
  • Police and fire personnel, correctional staff
  • Education and child care
    • All staff in regulated child care, public and private school programs, out-of-school time programs, virtual learning support, and community learning center programs.
    • All staff in Boys and Girls Clubs and YMCAs.
    • All staff in preschool and Head Start through K-12 education settings.
    • Faculty and staff in higher education settings who have direct student contact
  • Individuals enrolled in Medicaid long-term care programs
  • Some public-facing essential workers
    • Utility and communications infrastructure
    • Public Transit
    • Food supply Chain
      • Agriculture production workers, farm owners and employees
      • Workers on-site at agriculture operations, such as livestock breeding and insemination providers, farm labor contractors, crop support and livestock veterinarians.
      • Food production workers
      • Retail food workers, such as grocery store, convenience stores, and gas stations that sell groceries
      • Hunger relief personnel, charitable food distribution
  • Non-frontline essential health care personnel
  • Facility staff and residents in congregate living settings

You do not fall in any of the other eligible categories. Due to limited vaccine availability, please do not register or call to receive your COVID-19 vaccine if none of the eligible criteria apply to you. We appreciate your patience and assure you that everyone that wants a COVID-19 vaccine will get one.

In the meantime, it is critical we continue to practice COVID-19 safety measures as we have for the past several months; wear a mask, physically distancing, frequent hand hygiene.

*Please Note: Eligibility doesn’t guarantee availability. Although you may be eligible to receive the vaccine, the availability of vaccine may still be limited and you may not be able to receive your vaccine right away.

Are you currently in quarantine or isolation due to being COVID-19 positive or in contact with someone being COVID-19 positive? *

Please wait until quarantine or isolation has been cleared before scheduling for the vaccine.

Have you ever used the Oneida Comprehensive Health Division (Medical, Dental, Optical, Behavioral Health, Pharmacy, Physical Therapy or AJRCCC)? *

If you are experiencing COVID symptoms or are currently in quarantine you may not schedule a vaccine appointment at this time.
