FOODSHARE In order for clients to qualify, they must have already received their FS benefits at the time they are applying for replacement (you can verify this in the FS issuance history tab). They will need to complete the attached form and submit verification of their outage which could be: information provided by the fire department, the police, a community organization, or other sources of help (such as a statement from utility provider listing dates of outage). Clients will have to submit the form and verification to their local agency -the agency will not provide the verification on behalf of the client. Email dsmith1@oneidanation.org for questions.
